Cardiac Rehab
Personalized Programs to Improve Heart Health
The cardiac rehab program at Horizon Health is specifically designed to
meet the individual needs of each patient recovering from a variety of
cardiac conditions occurring in the last 12 months. Conditions include:
- Heart failure
- Heart attack
- Coronary artery bypass surgery
- Stable angina pectoris
- Heart valve repair/replacement
- Heart or heart-lung transplant
- Angioplasty
- Arrhythmia
There are three phases of cardiac rehab:

Phase 1 - Rehabilitation takes place in the hospital, immediately following a
procedure or event. (Not available at Horizon Health)

Phase 2 - Begins after hospital discharge and focuses on aerobic exercise, education,
and counseling for 12 – 36 weeks. Phase 2 is staffed by our cardiac
nurses who monitor patients via telemetry, take vitals, and instruct in
safe exercise progression utilizing the rehab department equipment.

Phase 3 - Supervised cardiac weight training and aerobic exercise following Phase
2, with vitals monitored throughout exercise.
