Our medical needs increase as we get older, and it’s important to have a medical team that is familiar with these needs and will support you or your loved one. Your first visit to Senior Care will consist of a physical and mental health evaluation, medical history review, and discussion of treatment options going forward. Getting older comes with many stressful challenges such as witnessing passing, leaving one’s own home to live in an assisted care facility, and dealing with your own increasing medical needs. At Horizon Health, we take a holistic approach to senior care, taking the time to address a patient’s physical, mental, and emotional needs.
