Cardiopulmonary Rehab is a supervised exercise program that helps people
with heart, lung, and vascular diseases live healthier, more active lives.
We provide a personalized treatment plan that combines structured exercise,
education, nutritional counseling, stress management, and other lifestyle
changes. We offer a friendly group environment where patients learn to
cope with their condition and improve their quality of life.
A medical referral is required to participate in the program.
Cardiopulmonary rehab combines the benefits of cardiac and pulmonary rehab

SilverSneakers group exercise– Horizon Health is a participating member in SilverSneakers,
a national health and fitness program for people age 65 and older. It
is free to seniors who have eligible Medicare or commercial insurance
coverage. Classes include chair exercises, stretching/balancing movements,
and strength training (resistance bands and handheld weights). Registration
is required as space is limited. To determine your eligibility for SilverSneakers,

Insurance Coverage
When it comes to deciding how to pay for cardiopulmonary rehab, our team
will review your existing insurance coverage and determine your plan’s
eligibility. Cardiopulmonary Rehab is generally covered by most insurance
plans for a diagnosis of heart attack, heart failure, coronary bypass
or valve surgery, chronic obstructive pulmonary disease (COPD), chronic
bronchitis, emphysema, lung cancer, peripheral artery disease (PAD), and more.
