From individual training sessions to our specialized Momentum program, we educate athletes on proper nutrition, hydration, and conditioning techniques to optimize performance and prevent injuries. We collaborate with coaches, physicians, and other healthcare professionals to create comprehensive care plans for all athletes.

We help school-age athletes return to school and sports safely after a concussion in accordance with state guidelines and assessment protocols. Our trainers work closely with school nurses/designated staff to modify school activities during this return-to-learn, return-to-sports process.
A free injury evaluation is available to athletes participating in junior high school, high school, or travel athletics. Our certified athletic trainers provide a detailed home exercise program, one follow-up injury check, and a return to sports plan. Call the rehab department at (217) 466-4244.
Looking for ways to improve your running technique? The running analysis program at Horizon Health pairs athletes with physical therapists who help them elevate their technique.
A licensed physical therapist will record, and subsequently analyze, an individual’s biomechanics while running on a treadmill. Video is assessed frame-by-frame to determine abnormalities, which can then be addressed through skilled physical therapy sessions and individualized home exercise programs.

For Horizon Health to provide sports medicine services to a student athlete, a parent or legal guardian must complete a Consent for Treatment form before the start of the sports season. The form needs to be completed once per year and DOES NOT need to be done for every sport.
A printed form should be completed and returned to the Horizon Health Rehabilitation Services department.
This form is completed at the direction of Horizon Health athletic trainers ONLY. The form applies after a concussion occurs and involves an athlete’s return-to-play/return-to-learn.
