Welltok Breach 2023
Horizon Health is one of thousands of companies that was involved in a
cyber incident involving personal information. We want to provide our
patients with details of the attack and resources you can use to protect
your information.

Letters were mailed to all affected patients on November 17, 2023, from Welltok.

- You are being offered free credit monitoring by Welltok, the third-party vendor. Please ensure you enroll by following the steps outlined in your personalized letter. The offer expires February 29, 2024.
- We have no indication that the data obtained has been used for fraud or other ill intent.

At Horizon Health, we take the privacy and security of information entrusted to us seriously. We deeply regret that this incident occurred. Within the last two years, Horizon Health has spent roughly $1 million on cyber security, including external audits, additional security for our internal networks, training for our staff, and monitoring solutions to protect against future cyber-attacks. We intend to continue that diligence.
