Patient Rights & Responsibilities

As a patient, you have the right to:

  • Care that is considerate and respectful regardless of age, race, color, ethnicity, sex, religion, physical or mental disability, socioeconomic status, national origin, language, sexual orientation, gender identity, gender expression or ability to pay.
  • Effective communication with any patient who has vision, hearing, or cognitive impairments in a manner that meets the patient’s needs at no cost.
  • Be fully informed in a language that he or she can understand and provide access to FREE translation services & auxiliary aid services as needed.
  • Request reasonable modifications for individuals with disabilities.
  • Ask that a family member or representative of your choice and your own Physician be notified promptly regarding your admission or discharge.
  • Make decisions and be involved with implementation of the plan of your care.
  • Obtain information as needed so you may give or refuse consent for any planned procedure or treatment. Information should contain any risks, problems, or side effects that may result.
  • Refuse care, treatment, and services to the extent permitted by law and hospital policy and to be informed of medical consequences of this action.
  • Confidentiality and personal privacy.
  • Prepare advanced directive such as living will or durable power of attorney for health care and receive care as it designates.
  • Have visitors regardless of age, race, sex, religion, disability, national origin, or sexual orientation.
  • Review medical record and have records explained.
  • Ask questions or voice concerns and file a complaint to staff member or management. You may contact Risk Management at (217) 466-4516. Complaints will be processed promptly.
  • Refuse care by students in training.
  • Understand and be informed of reasons for transfer to another facility
  • Be free of restraints unless it is only for means of safety.
  • Ask for a second opinion.
  • Ask questions about anything you do not understand.
  • Be free from harassment, neglect, exploitation, corporal punishment, and abuse while receiving care.
  • Access protective services.
  • Appropriate assessment and management of pain.
  • Be informed of alternative care and treatment when hospital care is no longer appropriate.
  • Request and receive a printed copy of your hospital charges and have them explained.

As a patient, you have the responsibility to:

  • Follow your Physician's Instructions.
  • Report any changes in your symptoms, medication or general condition to your physician.
  • Accept outcomes of refusing treatment or failing to follow care, service or treatment plan.
  • Follow hospital rules, regulations and policies during your stay.
  • Notify hospital management of any complaints of your care.
  • Answer questions accurately and honestly.
  • Follow our non-smoking policy. Horizon Health is a tobacco-free campus. Smoke/vape/tobacco not permitted
  • Treat staff and other patients with respect. Verbal aggression will not be tolerated and physical assault will be reported to law enforcement.
