Horizon Health Announces Plans for Jasper Clinic

Beginning October 1, many visiting specialist providers at Horizon Health will see patients at a new clinic location in Paris.

This move applies to patients of Dr. Nasser (cardiology); Dr. Verma, Dr. Gera, Megan Temples, nurse practitioner, and Dr. Jeevan (nephrology); Dr. Huh (oncology); and Dr. Satar (ophthalmology).

Offices for these providers will move from the Acklin-Shackelford Clinic to the Jasper Clinic at 15325 US Highway 150 Suite B (former Dollar General).

As a reminder, these visiting providers are not employed by Horizon Health. Phone communication, appointment scheduling, and needs regarding health records will process in the same manner as the past—patients must contact their home offices directly.

The Jasper Street Annex also houses the Horizon Health Pain Management Clinic and Health Information Department (medical records).

The Horizon Health Occupational Health Clinic, which previously was housed within the annex, is now located in the lower level of the EZ Care Paris building at 1 Phipps Lane. The phone number has not changed – (217) 466-4258.
