Three New Providers Join EZ Care Staff

Three new providers have joined the staff at EZ Care, a service of Horizon Health.
They are nurse practitioners Michelle Miller, Mallory Cook, and Charlene Moore. Together they have 27 years of nursing experience and a combined eight and a half years as nurse practitioners. Each has significant emergency department experience, including certification in advanced trauma life support.
“Their backgrounds in emergency medicine and trauma care is very impressive and will be an added benefit to our patients,” said Carrie Cunningham, EZ Care manager. “Their experience will provide patients with an added level of comfort often seen in an emergency room setting and allow us to offer a broader range of services, such as splinting of orthopedic injuries, care of more complex lacerations, and treatment of abscess. We are very fortunate to have them as part of our EZ Care team.”
The three providers bring a “wealth of experience” to Horizon Health, Cunningham said.
- Mallory Cook, of Marshall, earned her bachelor’s and master’s degrees in nursing at Indiana State University. Her experience includes working as an emergency room nurse/nurse practitioner in Terre Haute. She became a nurse practitioner in August 2017.
- Michelle Miller, of West Terre Haute, earned both her bachelor’s and master’s degrees in nursing at Indiana State University. Her experience includes working as a registered nurse in a neonatal intensive care unit, and as a nurse practitioner in an emergency department in Terre Haute, a position she held since becoming a nurse practitioner in May 2011.
- Charlene Moore, of Marshall, earned her bachelor’s and master’s degrees in nursing at Indiana State University. She worked as a registered nurse in an intensive care unit and as a nurse practitioner in an emergency department in Terre Haute. She became a nurse practitioner in December 2017.
Miller, Moore, and Cook join the EZ Care staff that includes: Carrie Cunningham, family nurse practitioner and EZ Care manager; Sneha Patel, family nurse practitioner; Jackie Coffman, physician assistant; Jolena Martin, physician assistant, and Matt Woodruff, physician assistant.
EZ Care treats minor, non-emergency health conditions such as ear, nose, and throat infections; flu symptoms; pink eye; rashes; minor lacerations; simple fractures; and burns. Some minor procedures can be performed.
EZ Care is open Monday through Friday from 9 am to 7 pm, and Saturday and Sunday from 10 am to 6 pm. The clinic is open 362 days a year, closed only on Easter, Thanksgiving, and Christmas.
